Wholesale

If you would like to apply for a wholesale account with Mellyn's please fill out the below form and under comment include your business name, address, and tell if you are a store front, online seller or both.  When I receive it I will send you my wholesale requirements and a short form to fill out.  Once you are approved I will send you instructions on how to order.
